What You'll Be Doing:
Conduct thorough psychosocial assessments to determine clients' mental health needs.
Develop and implement individualized treatment plans in collaboration with healthcare teams.
Establish and maintain therapeutic relationships with patients, guardians, family members, and significant others.
Provide individual, family, and group therapy using a variety of established therapeutic techniques.
Ensure ongoing communication about discharge issues with the treatment team, patients, and families.
Advocate for clients to ensure they receive appropriate resources and support.
Coordinate with other healthcare providers to deliver comprehensive care.
Offer crisis intervention services during psychiatric emergencies.
Educate clients and families about mental health conditions, treatment options, and available resources.
Maintain accurate and confidential client records.
Facilitate access to community resources and support services.
Monitor and evaluate clients' progress and adjust treatment plans as needed.
Assist with transportation services for patients to appointments.
Initiate or oversee guardianship and/or involuntary commitment proceedings.
Monitor legal issues such as probation or parole involvement and court hearings.
Provide support and modeling to other hospital staff.
Provide clinical analysis and recommendations at diagnostic and treatment review conferences.
Assist in covering social service needs as they arise.
What You Bring to the Table:
Minimum of one year of clinical experience in direct patient care as a Licensed Clinical Social Worker.
Experience with pediatric and adult patients.
A Master's degree in Social Work (MSW) from an accredited program.
Current licensure in New Hampshire with no record of disciplinary action.
Basic Life Support (BLS) Certification from an approved organization.
Ability to read, write, and speak English effectively.
US Citizenship or Permanent Resident Card.
Ability to reliably commute or relocate to Concord, New Hampshire.
Ability to travel on short notice or serve in various medical units.
